Empowering Clients with Long Term Conditions Through My New Qualification

 Diploma in Supporting Participation in Physical Activity : Long Term Conditions

I am now fully certified to accept clients with managed and controlled long term conditions without the need for a formal GP referral.


In today’s health and fitness world, it’s no longer enough to just help clients lose weight or build muscle. Nearly half (48%) of UK adults live with at least one long-term condition (LTC)—and one in five are living with two or more. Conditions such as diabetes, cardiovascular disease, musculoskeletal disorders, mental health challenges, and cancer are part of the lived reality for millions.


Motivation Behind Pursuing the Qualification

  • Desire to improve support and outcomes for new & current clients with long-term conditions.

  • Identifying gaps in previous care or knowledge.

  • Clearly linked my knowledge and experience from previous nursing & health promotion roles to my current post as a fitness professional.

  • Personal and professional growth.

  • Commitment to ongoing professional development.


Education for supporting clients with LTCs has had a significant overhaul in recent years. The previous GP exercise referal course mainly focused on GP referal schemes and only included a few LTC groups. The new course is far broader and includes 10+ groups.


Why it matters:

Working with clients who have LTCs is about more than adapting exercises—it’s about improving quality of life, building confidence, and supporting holistic wellbeing. Fitness professionals trained in this area are better equipped to:

  • Design safe, effective programmes tailored to specific conditions

  • Support clients managing multiple health challenges

  • Recognise when to refer to other professionals

  • Deliver inclusive, evidence-based coaching that empowers every client


For me, the most valuable difference is the shift in autonomy. Previously the onus was on the GP to source an accredited fitness professional and signpost accordingly.

Rather than having to try to make an appointment to obtain an 'exercise prescription', adding pressure to an overstretched NHS, this course removes barriers and makes it easier for people to confidently and safely take steps to start improving their health immediately.
